Notice 01 — Notice of Privacy Practices (HIPAA)

This notice describes how Longevita Nutrition Partners collects, uses, and protects your health information, and explains your rights regarding that information.
Who We Are
Longevita Nutrition Partners (Longevita LLC) is a registered dietitian nutrition practice operated by Michelle Shamailov, RD, LD. We are a HIPAA-covered entity and are required by federal law to maintain the privacy of your Protected Health Information (PHI) and to provide you with this notice.
What Information We Collect
We may collect and maintain information that includes your name, contact details, date of birth, health history, dietary assessment data, nutrition care plans, communications with your care team, and billing information. This information is stored securely within our HIPAA-compliant electronic health records system (Practice Better).
How We May Use and Disclose Your Information
Treatment. We use your health information to provide nutrition counseling, develop individualized care plans, and coordinate care with collaborating physicians or other health professionals — but only with your written authorization.
Business operations. We may use de-identified, aggregate data for quality improvement purposes. No individually identifiable health information will be used for marketing without your explicit written consent.
As required by law. We may disclose your information when required by federal, state, or local law, including mandatory reporting obligations and in response to lawful court orders or subpoenas.
We will not sell your health information, disclose it to third parties for marketing purposes, or share it in any way not described in this notice without your written authorization.
Your Rights
Access — Request a copy of your health records in our possession.
Amendment — Request corrections to records you believe are inaccurate or incomplete.
Restrictions — Request limits on certain uses or disclosures of your information.
Confidential communications — Request that we contact you by a specific method or address.
Accounting — Request a list of disclosures made in the past six years.
Complaint — File a complaint with us or with the U.S. Department of Health & Human Services. You will not be retaliated against for filing a complaint.
Telehealth Confidentiality Limitation
While we use HIPAA-compliant technology for all virtual sessions, no electronic system can guarantee absolute security. By engaging in telehealth services, you acknowledge that there is an inherent risk of unintended access to electronic communications. We take all reasonable precautions to minimize this risk. This disclosure is required by 16 TAC §116.105(b)(6).
Changes to This Notice
We reserve the right to change this notice and to make the revised notice effective for health information already collected. Any updated notice will be posted on our website with a new effective date. You may request a paper copy at any time.

Notice 02 — Your Right to a Good Faith Estimate
No Surprises Act, effective January 1, 2022
Plain-Language Summary
You have the right to receive a written cost estimate before your services begin. Longevita Nutrition Partners is a transparent, cash-pay practice. There are no surprise bills.
What This Means for You
Under the No Surprises Act, you have the right to receive a written Good Faith Estimate (GFE) of expected charges before receiving services from Longevita Nutrition Partners.
All rates are posted on our website during booking. There are no hidden fees, no insurance billing, and no surprise statements after your appointment.
Dispute Resolution
If your final bill is $400 or more above the amount listed in your Good Faith Estimate, you have the right to dispute the bill. Please contact us first — we will work to resolve any discrepancy promptly. You also have the right to initiate a Patient-Provider Dispute Resolution process through the federal government.

All nutrition counseling services at Longevita Nutrition Partners are provided by Michelle Shamailov, a Registered Dietitian (RD) credentialed by the Commission on Dietetic Registration (CDR) and a Licensed Dietitian (LD) in the State of Texas, regulated by the Texas Department of Licensing and Regulation (TDLR).

Scope of Practice
Services provided by Longevita Nutrition Partners constitute medical nutrition therapy and nutrition counseling within the scope of dietetics practice. These services are not a substitute for the care of a licensed physician or other medical provider. Longevita Nutrition Partners does not diagnose medical conditions, prescribe medications, or provide medical treatment.
Notice 04 — Telehealth Services & Geographic Scope

We serve clients virtually and in person. Virtual services are available only to clients who are physically located in Texas or Michigan at the time of service.
How We Deliver Services
Longevita Nutrition Partners provides nutrition counseling both virtually (telehealth) and in person in the Austin, Texas area. Virtual sessions are conducted via secure, HIPAA-compliant video technology through Practice Better.
Where We Are Licensed to See You
Under federal and state law, telehealth services are governed by the laws of the state where the client is physically located at the time of service — not where the provider is located. We are only able to provide telehealth services to clients who are physically present in a state where our practitioner holds an active license.
We are currently licensed to provide telehealth services to clients located in:
Texas — Virtual and in-person services available.
Michigan — Virtual services only.
If you are unsure whether we can serve you, please contact us before scheduling.
Your Responsibilities During Telehealth Sessions
Be physically located in Texas or Michigan at the time of each virtual session.
Ensure a private, confidential environment on your end during sessions.
Have a stable internet connection and a device capable of video conferencing.
Notify us in advance if you will be traveling and located in a different state than usual.
Technology and Security
All telehealth sessions are conducted through Practice Better, which uses end-to-end encryption and meets HIPAA technical security requirements. In the event of a technology failure, we will attempt to reach you by phone to reschedule or continue by audio if clinically appropriate.
Superbill Policy
Longevita Nutrition Partners is a cash-pay practice and does not bill insurance directly. Upon request, we will provide a superbill — a detailed receipt containing diagnosis codes, procedure codes, and provider information — that you may submit to your insurance carrier for potential out-of-network reimbursement. We make no guarantee of reimbursement, as coverage decisions are made solely by your insurer. Payment to Longevita Nutrition Partners is due at the time of booking regardless of your insurance status or reimbursement outcome.
