In-office partnership services
For clinics seeking a more integrated arrangement, Michelle offers a limited number of recurring on-site partnerships. Patients are seen in person in your office, with cases discussed in real time and a written assessment provided to your team after each visit.

Pre-op nutritional risk and frailty assessments
Using validated assessment tools, Michelle will stratify your surgical patient's nutritional and frailty risk, providing either clearance or suggested target goals and an individualized prehabilitation plan.
Nutrition consultations
Individual nutrition assessments, education, and care plans conducted in your office alongside your clinical visit.
Group classes
Single sessions or series, on-site or virtually. Content is tailored to your patient population and co-branded with your clinic.
Professional consulting and staff education
Nutrition is addressed in medical offices daily — feel confident in the advice and materials that your team is sharing. Examples include lunch & learns or creation of custom educational handouts.
A note on in-office partnerships:
Longevita operates as an independent contractor. Patients must sign Longevita’s consent to record sharing prior to any case-specific discussion or notes. All patient services are billed directly to patients through Longevita LLC. Alternative billing arrangements, including clinic-bundled services (example: including a dietitian visit at your clinic as part of a surgical package), can be discussed on inquiry.
