Direct patient referral

Coordinated referral
Your office can send a referral note via HIPAA-compliant fax or secure e-mail. Upon request, we will follow up with our assessment and clinical summaries of our visits — provided the patient has consented to record sharing.
In-office and partnership services
For clinics seeking a more integrated arrangement, Michelle offers a limited number of recurring in-person, on-site partnerships in the greater Austin area. Cases are discussed in real time with a written assessment provided to your team after each visit, with patient consent.

Pre-op nutritional risk and frailty assessments
Using validated assessment tools, Michelle will stratify your surgical patient's nutritional and frailty risk, providing either clearance or suggested target goals and an individualized prehabilitation plan.
Nutrition consultations
Individual nutrition assessments, education, and recommendations conducted in your office alongside your clinical visit.
Group classes
Single sessions or series, on-site or virtually. Content is tailored to your patient population and co-branded with your clinic.

A note on in-office partnerships:In-office services are available to a limited number of clinic partners, depending on location and scheduling. All patient services are billed directly to patients through Longevita LLC. Alternative billing arrangements, including clinic-bundled services, can be discussed on inquiry.
Longevita operates as an independent contractor. Patient consent for record sharing is obtained prior to any clinical communication with your practice.
